Clinical Question
What common medicines have serious interactions with cannabidiol?
Bottom line
CBD, a nonintoxicating phytocannabinoid, is widely available and may interact with other medications. Although a number of interactions have been documented, their likelihood and impact have not yet been described. Consider asking patients about their CBD use before prescribing another psychoactive medicine and ask if they have experienced any unexpected side effects with other medications. 5

Synopsis
These authors searched 3 databases, including a drug-interaction database, to identify drug interactions with cannabidiol (CBD). In addition to affecting the brain, CBD also affects other organs and systems, including the liver. The list of interactions is large, and this synopsis presents only the common medicines with significant interactions. CBD inhibits the hepatic enzyme CYP2D6 and may increase the blood levels of selective serotonin reuptake inhibitors, tricyclic antidepressants, antipsychotics, beta-blockers, and opioids. CBD will increase serum levels of lamotrigine (Lamictal), but other anticonvulsants will decrease serum levels of CBD.

DDI with cannabidiol and commonly used meds
Although I dont prescribe cannabidiol I wonder to what extent other cannabinoids, like those in recreational products also interact with common meds that I do prescribe

CBD interaction with other medications
Since CBD is widely used without prescription, this is a potentially very important piece of knowledge to know when taking a medication histroy
CBD Interactions - Mostly Not Clinically Significant
While there are a great many *potential* drug-drug interactions (DDIs) with CBD, there are actually very few that are clinically relevant - primarily clopidogrel, clobazam, and warfarin. Drugs that interact with the Cytochrome p450 system and reduce the plasma levels of CBD are less of an issue, as the personalized dosing and fine dosing adjustments that can be made with CBD allow for easy compensation should a new medication be started.
