Chronic inflammation caused by textured breast implants may lead to a malignant transformation of T cells and an increased risk of lymphoma. These investigators retrospectively reviewed all patients undergoing implant-based breast reconstruction from 1991 through 2017 at a single US institution (Memorial Sloan Kettering Cancer Center). The individuals who assessed the data from surgical and medical records, as well as the data from pathological specimen reviews, were not specifically noted as being masked to the study hypothesis. During the 26-year study period, complete record reviews occurred for 9373 patients undergoing breast reconstruction surgery with the placement of 16,065 implants. Eleven cases of BIA-ALCL occurred among study patients with a median time to diagnosis of 10.26 years (range 6.43 - 15.52 years). The overall incidence of BIA-ALCL in patients breast reconstruction surgery with placement of textured implants was 1 in 559 patients and 1 in 871 implants. No cases of BIA-ALCL developed in patients who had only smooth implants placed. However, there were significantly more patients lost to follow-up in the smooth implant group.
