Are primary care interventions effective in preventing or ceasing tobacco use in school-aged children and adolescents?

In this updated 2020 review, the United States Preventive Services Task Force (USPSTF) found adequate evidence that behavioral counseling interventions, print materials, and computer applications are effective in preventing the initiation of tobacco use (including e-cigarettes) in school-aged children and adolescents (B recommendation). However, the task force found insufficient evidence of benefit from primary-care feasible interventions for the cessation of tobacco use among school-aged children and adolescents who already smoke (I statement). These recommendations are consistent with the 2013 USPSTF recommendations. 1a

In this updated review, the task force found sufficient evidence of benefit from behavioral interventions in decreasing the likelihood of tobacco use in children and adolescents. Behavioral interventions include print materials, computer applications, and face-to-face or telephone interactions with a health care clinician. In addition, the task force found no evidence of harm from behavioral counseling interventions for the prevention or cessation of tobacco use. There was no significant evidence of benefit, however, for behavioral interventions, medications, or nicotine replacement therapy for tobacco cessation among children and adolescents who already smoke. No medications are currently approved by the FDA for tobacco cessation in children and adolescents. The American Academy of Pediatrics and the American Academy of Family Physicians recommend counseling to all children and adolescents to prevent tobacco use initiation.
