In this updated 2019 review, the U.S. Preventive Services Task Force (USPSTF) recommends that clinicians screen for hepatitis B virus (HBV) in pregnant women at their first prenatal visit (A recommendation). This recommendation reaffirms the 2009 USPSTF recommendation statement. 2c

The task force previously found adequate evidence that available HBV serologic tests are highly accurate in diagnosing HBV infection. In addition, current interventions—including vaccination of all infants against HBV infection and providing postexposure prophylaxis with hepatitis B immune globulin at birth to infants of mothers infected with HBV—are effective for preventing perinatal transmission and subsequent chronic HBV infection. Although no studies were found that directly evaluated HBV screening vs no screening, observational studies have shown that, over time, perinatal transmission has decreased among women and infants enrolled in screening and case management. The task force found no evidence of direct harm from screening. The Centers for Disease Control, the American College of Obstetricians and Gynecologists, and the American Academy of Family Physicians recommend screening for HBV infection in pregnant women at their first prenatal visit.
